Case Management Services
Our Case Management Services help individuals and families achieve wellness and autonomy by providing advocacy, comprehensive assessment, planning, communication, health education, resource management, service facilitation and using evidence-based guidelines with a participant-center approach.
- Community Navigation: We support individuals in recognizing and pursuing their distinct interests and objectives. Our assistance includes helping them discover relevant opportunities within their local communities that align with their unique aspirations and connecting them with others who share similar interests and goals. Additionally, we address social determinants of health by linking individuals to local organizations and facilitating access to essential services, such as social services, benefits, health insurance, food programs, affordable internet, and various health services, including dental care, mental health support, primary care, and chronic disease management, among others.
- Assistance with Applications: SNAP, WIC, Housing,etc.
- Job Preparation/ Support: We provide guidance and direct individuals to career development programs that help them prepare for job interviews and discover employment opportunities at no cost to individuals.
- ARTS Program: We provide referrals to our community partners’ ART Program at no cost to individuals.
- College Preparation: We provide referrals to our community partners' Career Development and College Preparation Program at no cost to individuals.
- Business StartUp Coaching: We offer comprehensive guidance throughout the process of establishing both Nonprofit and For-Profit organizations.
- Education: We offer information and training to enhance understanding of social issues.
- Life Coaching: We assist individuals in reaching their personal and professional objectives by offering support, fostering accountability, and implementing a structured approach to facilitate positive transformation.
- Legal Support: We offer assistance in locating local resources in your community that provide legal-specific services. However, we do not offer legal advice.
- Social Intervention: We provide guidance to individuals by assessing their needs, helping them discover available programs, and making referrals. We maintain ongoing engagement to ensure that social challenges are effectively addressed.
